PS-PK Camper File

Since your child is enrolled in the PS/PK group, we’ll be requesting some additional documentation to ensure a safe and smooth experience for all.

• If your child is a current student at our school, their school file will automatically roll over for summer camp—no action is needed on your part.

• If you’re new to our school, please complete the attached checklist of required items. Note that two of these forms will require a physician’s visit, so we recommend scheduling appointments as soon as possible.

Please note that your camper will not be allowed to start camp until all items on the checklist have been received. This policy ensures we can provide the safest and most supportive environment for every child.
